Staff may complete the Request for Reasonable Accommodations form and submit to the direct supervisor who will review with the Clinical Director and Operations Director. If an accommodation is deemed unreasonable, this will be explained confidentially to the staff member making the request and all efforts will be made to find a reasonable alternative.
